How to Enroll!
Fill out an online form here:
If you would prefer to apply in-person, you can make an appointment by calling the office in advance, however walk-ins are always welcome. Please bring the following documents to the CAAS office to complete an application:
Proof of Child’s Age (passport, birth certificate, etc.)
Proof of Family Income (Tax form, one month of pay stubs, DTA letter, etc.)
Proof of Family Residency (any bill that has been mailed to you)
Child’s Health Insurance Card
Please bring all documents to our offices at 66-70 Union Square, Suite 104, Somerville, MA.
